Southwest Health System offering sports physicals for the upcoming school year

Southwest Health System will perform sports physicals at various locations in preparation for the upcoming school year.

Physicals cost $35, and all local districts are welcome to utilize the locations, according to a news release from Southwest Health System.

Physicals will be offered at Montezuma-Cortez High School’s health clinic, Mancos Valley Clinic and the Southwest Medical Group in Cortez. Multiple dates and times are available to accommodate an array of schedules.

The school-based health clinic at MCHS will offer appointments on Monday, Aug. 5, Thursday, Aug. 8 and Friday, Aug. 9 from 8 a.m. to 4 p.m. at 418 S. Sligo St. in Cortez. Questions can be directed to (970) 564-4855.

Mancos Valley Clinic’s appointments are offered as walk-ins Wednesday, Aug. 7 from 8 a.m. to 5 p.m. The clinic will be closed noon to 1 p.m. for lunch. The clinic is at 111 E. Railroad Ave. in Mancos, and can be reached at (970) 533-9125.

Southwest Medical Group, 1311-A N. Mildred Rd. Suite B, also is offering sport physicals on Wednesday, July 24 and Wednesday, Aug. 14 with Dr. Moriah Tarpey. Appointments can be made with her office by calling (970) 564-2662.

Student athletes arriving for their appointment should have a signed sport physical form, signed parent consent form and method of payment (cash, card or check). Parents are not required to be present for the physicals.

Sports physicals can also be done at the child’s primary care provider or at the walk-in clinic in Cortez, Monday through Friday from 8 a.m. to 7 p.m. and Saturday through Sunday from 9 a.m. to 4 p.m.

The school-based health clinic will also provide well child checks on Tuesday, Aug. 6, by appointment only. To book an appointment, call (970) 564-4855 and leave a message. A staff member will return the call to schedule an appointment.
