Montezuma County health office seeks schools’ help with survey

The Journal

The Montezuma County Health Department announced on Monday that it is collaborating with unnamed “community shareholders” to conduct a health assessment survey.

“We will be running the survey from Nov. 1–Nov. 15,” Melissa Mathews, of the health department, said in an email to staff at Montezuma-Cortez High School. “If you would, please share with your staff, middle school and high school parents.”

The online survey asks users to “select/rate the top three issues in order of importance in your life,” and which age, racial and economic groups would most benefit from services. The survey also asks participants to include information about themselves, such as age, race, education level, information sources and contact information.

The survey is at www.surveymonkey.com/r/MontezumaCountyHealthSurvey.
