Home health care provider PASCO/SW is relocating offices in Cortez and Durango, according to a news release.
The Cortez offices have moved to Cortez Plaza at 2208 Main St. from its previous South Washington Street location.
The new office is on the left side of shopping center between Maley & Associates Inc. and the Colorado Department of Driver License office.
The Cortez PASCO/SW office will host all administrative and business staff members across their diverse functions in home health, home care and Hospice.
In addition to the Cortez headquarter relocation, effective Aug. 29, the Durango office moved to 1075 East Main Avenue, Suite 223 from its previous location on Second Avenue. The new business suites are connected left of Alpine Bank in the Crossroads Clocktower building.

The Home Health Care agency has grown exponentially in the past year with over 120 employees. It also has an office in Grand Junction.

PASCO/SW was founded in 1999 as a sister agency to PASCO in Denver, then split into an independent agency in 2006. In 2021, PASCO/SW, which stands for Personal Assistance Services of Colorado Southwest, became part of the Cornerstone Inc. organization to extend its resources and community reach.
The home health provider supports the community through skilled home health and non-medical home care services while utilizing the most prevalently known insurances in the region.
These health care coverages include Medicare, Veterans Affairs, Medicaid, HCBS/IHSS Medicaid Home and Community Based Services, and most commercial insurances.
PASCO/SW serves nine counties, including Archuleta, Delta, Dolores, La Plata, Mesa, Montezuma, Montrose, Ouray, and San Miguel County.
Home health service is from a primary care provider that supplies skilled professionals like registered nurses, certified nursing assistants, physical therapists and assistants, occupational therapists and certified assistants, speech therapists, and medical social workers.
These services cover disease treatment and education, diabetic care and instruction, medication management, certified wound care, postoperative care, pain management, therapeutic and rehabilitative care. Services include home discharge planning, emotional support, and financial and community resources.
Home care can be a variety of non-skilled in-home services for those who want to maintain their independence and dignity by continuing to live in the comfort of their homes.
